This ENHANCE Consultation Guide is designed for clinicians – nurses and doctors – who work in primary care facilities and deliver care to people living with multiple long-term conditions.
The clinical recommendations
in the guide are drawn from the latest Practical Approach to Care Kit (PACK) 2021 and focus on supporting the adult patient and clinician through a routine visit for their chronic conditions.
Where appropriate this guide will refer the user to PACK for guidance for acute conditions, other chronic conditions not contained in the ENHANCE guide or for reproductive health care (pregnancy and contraception).

The guidance has been structured with a person-centred approach to help the clinician and the patient to work together to identify and address the patient’s health care priorities first with a view to supporting
patient self-management.
